Post-Acute Medical Care

When a hospital stay ends, it doesn’t always mean you’re at the same level of ability you were before you needed surgery, got sick, or were injured. If your recovery has reached a point where you’re healthy enough to be discharged from the hospital, but you still need more help than you have at home, St. Luke’s post-acute care can be the bridge to restoring some or all of your independence. Post-acute care can be either short or long-term, depending on your needs and pace of recovery. Our main focus is rehabilitation, helping you manage symptoms and improve function, allowing you to achieve your goals.

St. Luke’s providers partner with several skilled nursing facilities in the Treasure and Magic Valleys to provide a level of care that exceeds traditional past practices in nursing facilities to provide on-site availability for when care demands are highest. The St. Luke’s Post-Acute Medical Group provides timely evaluations to both our short-stay patients and our long-term care residents needing chronic care. We work to prevent visits to the emergency department or readmissions to the hospital in order to optimize your ability to live your best life.

In addition, St. Luke's post-acute care providers are on call 24 hours day, 7 days a week to assist in patient care needs.

Our team also collaborates closely with the administration of each facility to track quality of care, patient outcomes, and patient experience, and identify opportunities for improvement.

    Donna Beeson, DO is part of the St. Luke’s Post Acute Medical Group, focusing on taking care of patients in skilled nursing facilities after acute hospitalization. Her medical experience includes her own medical practice specializing in adult medicine; working as a hospitalist, caring for patients needing hospitalization; and caring for patients at long-term acute care hospitals, as well as in skilled nursing facilities as a hospice provider.  

    Dr. Beeson enjoys volunteering her time to read, play games, and talking with hospice patients throughout the Treasure Valley. She can be found volunteering at local elementary, middle, and high schools, and conducting sports physicals at Meridian High School. She has a passion for helping people in need and she supports community clinics and homeless shelters to provide care for those without health insurance.

    Lara Boyle, NP provides personalized care to residents in skilled nursing and long-term care facilities. She has specialized experience in asthma, allergy, and hospital medicine. Throughout her medical career, Lara has gained experience working in the inpatient, outpatient, and skilled nursing environment, providing her with insight in determining how to best set her patients up for achieving their health goals.

    Lara earned her bachelor’s degree in nursing science from Brigham Young University-Idaho, and her nurse practitioner doctorate from Idaho State University. She is certified by the American Association of Nurse Practitioners. In her free time, she enjoys gardening and playing with her dog.

    Erin Compton, RN, MSN, NP provides personalized care to residents in skilled nursing facilities. She is passionate about delivering a level of care that exceeds traditional past practices and works diligently to prevent visits to the emergency department. She is experienced as a provider and educator in emergency and cardiac medicine.

    Erin earned her master’s degree as an adult/gerontology primary care nurse practitioner at the University of North Carolina at Chapel Hill and is certified in trauma nursing. In her free time, she enjoys mountain biking, running, traveling, and hiking with her dog.

    Abby Daley, PA-C provides care to residents in both skilled nursing facilities and long-term care facilities. She is passionate about helping her patients work toward a successful transition back home. She also provides ongoing care and comfort to long-term care residents as they adjust to their new living environment.  

    Abby earned her bachelor’s degree in health sciences at Boise State University, and her master’s degree in physician assistant studies at A.T. Still University. In her free time, she enjoys running, hiking, cooking, and spending time with her husband and children.

    Matthew Dopp, MD serves patients as part of St. Luke’s Clinic – Internal Medicine, our hospitalist team, and the St. Luke's Post Acute Medical Group, managing patient care after a hospitalization as a medical director in skilled nursing facilities in the Magic Valley. 


    Dr. Dopp brings a positive attitude, a great bedside manner, and a comprehensive approach to caring for his patients. In his free time, Dr. Dopp enjoys baseball and spending time outdoors with his family.

    James R. Farmer, NP is part of the St. Luke's Post-Acute Medical Group and has extensive experience in treating patients in the post-acute care setting. Prior to his employment with St Luke’s Post-Acute Medical Group, he worked as a nurse practitioner providing post-acute care at St Luke’s Rehabilitation hospital. He has also spent time providing patient care to military veterans at the VA Boise Medical Center. 

    James earned teaching and nursing degrees from the University of Utah, and a master’s degree in nursing with an FNP designation from Idaho State University. In his free time, James enjoys hiking, camping, being outdoors, and spending time with his family.

    Originally from Idaho, Amanda earned her nursing degrees from Brigham Young University and her doctorate degree from Idaho State University. She is board certified by the AANP (American Academy of Nurse Practitioners). In her free time, Amanda enjoys kayaking, crafting, and playing the piano.

    Jessica Gratzer, NP is a nurse practitioner with St. Luke’s Post Acute Medical Group, and has experience in the intensive care unit, emergency department, and in surgical settings. She has a passion for end-of-life care, including completing advanced directives. She works to keep her patients out of the hospital and to coordinate care for transitioning home.

    Jessica earned her bachelor’s degree as a nurse practitioner from Boise State University. She is a member of the American Association of Critical Care Nurses and the Gerontological Advanced Practice Nurses Association. In her free time, she enjoys time with her family, reading, rock hounding, and getting to the mountains. Jessica also volunteers at Jerome and Twin Falls soup kitchens as well as handing out books to underprivileged youth.

    Kristina Liebenthal, RN, MSN, FNP, CNN-NP is part of St. Luke's Post-Acute Medical Group, and a board certified nurse practitioner who has worked primarily in the fields of nephrology and hospital medicine. Her professional interests include mentoring new students and new nurse practitioners, and working to raise patient awareness about prevention and treatment of chronic kidney disease.

    Kristina is a member of the American Academy of Nurse Practitioners, Nurse Practitioners of Idaho, and National Kidney Foundation's Council of Advanced Practitioners.

Away from St. Luke's, Angela has participated in international medical missions to Bolivia and Guatemala.

    Angie earned her bachelor’s degree in nursing from Boise State University and her master’s degree as a family nurse practitioner from Walden University. In her free time, she enjoys traveling, camping, rafting, hiking, skiing, swimming, cooking, and reading with her family and friends. She also enjoys raising chickens.

    Lauren Meziere, PA-C is a certified physician assistant with St. Luke's Post Acute Medical Group. She earned her bachelor's degree from Georgia Institute of Technology, an MBA from the University of Phoenix, and her physician assistant training at Idaho State University. Lauren has a passion for participating in worldwide medical missions, volunteering to care for people in Ecuador and Zimbabwe. 

    In her free time, Lauren enjoys spending time with her family in the Idaho outdoors, skiing, hiking, camping, rafting, and fishing.

    Biography

    Stephen Montamat, MD is a member of a group of physicians, nurse practitioners, and physician assistants dedicated to providing the highest quality care to patients in skilled nursing facilities. He has a background in general internal medicine and geriatrics. He practiced in the clinic and hospital at St. Luke’s Internal Medicine for 23 years.

    Dr. Montamat was integral in the successful creation and implementation of St. Luke's Post-Acute Medical Group (PAMG) where he served as the medical director for four years. Under his leadership and direction, the program steadily grew throughout the Treasure Valley and Magic Valley promoting collaborative partnerships with community medical resources. He currently maintains a semi-retired status while continuing to support PAMG. 

    In his spare time, Dr. Montamat leads the educational course, Powerful Tools for Caregivers. He also enjoys reading, music, playing tennis, spending time with his wife, and playing with his dog.

    Robert Porter, PA-C is a physician assistant with St. Luke's Post-Acute Medical Group and has experience in emergency medicine, oncology, and hospital medicine. As a hospitalist, he developed a deep understanding of the presentation and treatment of complex medical conditions that cause hospitalizations. He now strives to optimize his patients' health to prevent readmissions and return them to the safest care setting. 

    Rob earned his bachelor's degree in zoology with an emphasis in human biology from Brigham Young University, and a master's degree in physician assistant studies from A.T. Still University in Arizona. He is certified by the National Commission on Certification of Physician Assistants and a member of the Society for Post Acute and Long-Term care.

    James Schrader, DO is the medical director for the Post-Acute Medical Group, caring for patients at skilled nursing and long-term acute care facilities. Dr. Schrader is passionate about assisting patients and their support systems during challenging times with high quality care, support, and collaboration. Prior to this work he worked in St. Luke’s Treasure Valley hospitals as a hospitalist for 10 years caring for patients needing hospitalization.

    Before attending medical school, Dr. Schrader was a Medical Service Corps officer in the United States Army for primarily at Ft. Lewis Washington. In his tenure he served in support of the 101st with the 62nd Medical Brigade in Iraq.  

    Dr. Schrader is an avid runner. He and his family enjoy taking advantage of outdoor opportunities including hiking, biking, fishing, and camping with his family and dog. He has a passion for helping people and volunteers to support veterans at local and regional events.

    Dr. Schrader is a member of the American Osteopathic Society and the Society for Post-Acute and Long-Term Care Medicine.

    Shannon Vaughn, MSN-FNP, CVNP-BC is a nurse practitioner with St. Luke's Post Acute Medical Group. She has experience caring for cardiac patients, and has expertise in general cardiology, internal medicine, congestive heart failure, atrial and ventricular arrhythmias, hypertension, diabetes, lipids, and cardiac device follow-up. She helped establish and support Faith & Humanity Medical Missions and co-led medical mission groups to Honduras, and also dedicates time to local communities, organizing annual women’s retreats as a Young Mom’s Mentor.

    Shannon earned her bachelor’s and master’s degrees in nursing from Idaho State University. In her free time, she loves to travel with family and friends.
