Preparing for an Appointment at St. Luke's

If you're seeing a provider for a new condition, a little preparation can go a long way:

  • Complete any paperwork sent by your provider before your visit.
  • For video visits, review our Prepare for a MyChart Video Visit guide.
  • Bring your medications (or a list of them), ID, and insurance card.
  • Track your symptoms in a diary—even those that seem unrelated.
  • Write down your questions ahead of time and bring them with you.
  • Consider bringing a support person to help take notes and ask questions.
  • Ask for clear explanations of your diagnosis, treatment options, and prognosis.
  • Don’t hesitate to seek a second opinion if you have doubts or concerns.
  • Arrive early to allow time for paperwork. We’ll keep you informed of any delays.
  • Please call at least 24 hours in advance to cancel or reschedule your appointment.
