We've made it easy to complete your Medicare Annual Wellness Visit! If you don't have a primary care provider or your established provider is unavailable, you can connect virtually with a St. Luke’s On-Demand Virtual Care provider at a time that's convenient for you. Services are available 7 a.m. to 10 p.m. Monday-Friday and 8 a.m. to 8 p.m. on weekends.

As a Medicare beneficiary, you're eligible for an annual wellness visit once every 12 months. 

Note: you must be physically located in Idaho, Oregon, Washington, Utah, Nevada, Montana, Arizona, or California at the time of your virtual care call.

Please review these instructions to ensure your visit goes smoothly.

What to Expect During Your Appointment

During your appointment, we'll cover many aspects of your health care, including:

  • Review your health risk assessment and current prescriptions.
  • Update medical and family history. 
  • Update the list of current providers and suppliers regularly involved in providing you with medical care or personalized prevention plans. 
  • Outline preventative screenings needed and how/where to complete screening. 
  • Review and update list of risk factors and conditions for which interventions are recommended or underway; provide personalized prevention plans. 
  • Discuss personalized health advice and provide referral, as appropriate, to health education or preventive counseling services or programs.  
  • Discuss advance care planning services and how you can let others know about your care preferences.
  • Guidance on any other health care questions.
