Share Your COVID-19 Vaccination and Testing Information

As you start to go back to work, travel, or attend events such as a concert or sporting event, you might be asked for COVID-19 vaccination information or recent test results. You can share your information from MyChart quickly, easily, and securely by:

  • Generating barcodes that can be scanned from your mobile device.
  • Downloading a PDF document that includes your vaccination barcodes. (Note: test result barcodes are not included in the PDF.)
  • Exporting your information to another application.

The barcodes that appear in MyChart are created according to an open industry standard called SMART Health Cards. They contain verified information and only what is relevant to COVID-19, including your legal name, date of birth, COVID-19 vaccination information, and your most recent COVID-19 test result. You decide how much to share and with whom—you can share only your test results, only your vaccination information, or both. Note

NOTE: Your vaccination status and the number of doses shown in MyChart will vary depending on if you have received a two dose series (Pfizer or Moderna), a single shot vaccine (Johnson & Johnson), or if you’re eligible for a third dose or booster. Although we’ll do our best to determine vaccine eligibility for a third dose based on the information in your medical record, please follow CDC vaccine recommendations to determine your eligibility for the third dose for moderately to severely immunocompromised patients, or a booster for patients at increased risk.

Generate a QR Code with Your COVID-19 Information

  1. Make sure you have the latest version of MyChart on your device. Check for updates through the app store. 
  2. Log in to your St. Luke’s MyChart account.
  3. Enable the ability to view COVID-19 information prior to log-in. To do this, find COVID-19 in MyChart's main menu. Under Quick Access Options, activate "View COVID-19 information on the pre-login screen." You’ll then be guided through the COVID-19 status preview. Once you’ve completed the preview, you’ll be asked if you want to turn on the COVID-19 status preview or keep it off. You must turn the status preview on to view your COVID-19 information without logging in to MyChart.
  4. Now, you can generate a QR code either from the MyChart home screen, or from the menu. Using the menu, go to My Record > COVID-19.
  5. Click QR Codes. If you have both COVID-19 vaccinations and test results on file, you'll have separate barcodes for each of them, and you can choose whether to allow someone else to scan only one or both.


Download a PDF Document with Your COVID-19 Vaccination Information

  1. Make sure you have the latest version of MyChart on your device. Check for updates through the app store. 
  2. Log in to your St. Luke’s MyChart account.
  3. From the menu, go to My Record > COVID-19.
  4. Select Download/Export and then Download vaccination PDF (on the MyChart mobile app) or Download (on the MyChart website). You can then save the PDF to your computer or mobile device and print if needed.



Export Your COVID-19 Information to Another App

  1. Make sure you have the latest version of MyChart on your device. Check for updates through the app store. 
  2. Log in to your St. Luke’s MyChart account.
  3. From the menu, go to My Record > COVID-19.
  4. Select Download/Export.
Note: With Apple’s recent iOS 15 version release, users can now add verifiable COVID-19 vaccination information as a vaccination card in Wallet and download and store verifiable vaccination and test result records in the Health app. View Apple Support: Add verifiable COVID-19 vaccination information to Apple Wallet and Health

If you're using the St. Luke’s MyChart mobile app: a series of screens appears to help you verify that you have another app installed on your device into which you can export your information. Then, another screen will show the data available for sharing. If you have both COVID-19 vaccinations and test results on file, a checkbox appears for each one. Select the checkbox for the information you want to share with the other app, and then click Export. Select the app you'd like to export your data into and follow the app's instructions.



If you're using the St. Luke’s MyChart website
: If you have both COVID-19 vaccinations and test results on file, a checkbox appears for each one. Select the checkbox for the information you want to share through the other app. Then, click Export. A copy of your information is downloaded to your computer or device. To import it into another app, go to that app on a mobile device and follow the app's instructions.


Not seeing your QR Codes?

  1. Make sure you have the latest version of MyChart on your device. Check for updates through the app store. 
  2. Log in to your St. Luke’s MyChart account.
  3. From the menu, go to My Record > COVID-19.
  4. Click Update to query Idaho’s Immunization Reminder Information System (IRIS)

    1. If you received your vaccine somewhere other than St. Luke’s, your immunization records are likely in IRIS and will be auto-reconciled when you click the Update button. Once auto-reconciliation is complete, your screen will look similar to the ones above and you can obtain your QR Code.
    2. If you just got your vaccine or if you received your vaccine out of state, you can submit your information through a quick questionnaire and upload a photo of your vaccine card. This will be manually reviewed by clinical staff and either approved or denied. Please see the steps below:


Access your QR Codes from Mobile Without Logging In

  1. Make sure you have the latest version of MyChart on your device. Check for updates through the app store. 
  2. Open the MyChart app on your mobile device.
  3. Click on QR Codes or Show Details (from the log in screen) to obtain the applicable information for either selection.
