Improving Health Through Patient-Centered Care

Caring people are the heart of St. Luke’s. They’re the reason the organization exists. Each member of the St. Luke’s team has a part to play in our strategy and is key to our success.  

St. Luke’s is driven by our mission to improve the health of people in the communities we serve, which is grounded in our focus on population and community health. Population health is the way in which St. Luke’s improves the health of people in the communities we serve by taking accountability for health outcomes and total cost of care. Community health is the way in which St. Luke’s, working in partnership with others, improves the health of people in the communities we serve by addressing social determinants of health.   

Our strategy leads us to achieving our vision as we build upon and enhance our reputation as the community’s trusted partner in providing exceptional, patient-centered care. 

St. Luke’s strategy follows a clearly defined path: We’re meeting people where they are on their health journey — improving health and lives by delivering exceptional performance and outcomes in safety and quality, access and affordability. In doing so, we are further solidifying St. Luke’s position as a community asset and a high-quality, independent, regional health system. 

By concentrating on the strategic focus areas of culture and workforce innovation, care management and primary care access over the coming years, St. Luke’s seeks to ensure delivery of the care patients need when, where and how they need it, through a connected and seamless experience.

The addition of specific focus areas — culture and workforce innovation, care management and primary care access — to St. Luke’s strategy underscores the organization’s commitment to prioritize and track progress where there is the expectation of greatest impact toward achieving St. Luke’s mission, vision and strategic objectives. Primary Care Access

We will improve access to preventive and routine care services for people in the communities St. Luke’s serves and in ways designed to meet the range of consumer expectations.
