Know Your Numbers (KYN) Screening for Spouses


At this screening, we’ll check your blood pressure, height, weight, BMI, waist circumference, and fasting blood glucose, and tobacco status. Participants with diabetes must provide an A1c lab result within 6 months of your spouse’s hire date.

Please be sure you bring your spouse’s St. Luke’s employee ID number to your screening.

2. Visit a Healthcare Provider Office

If you’ve completed an exam at a healthcare provider's office within six months of your spouse’s hire or benefits eligibility, download the Provider Verification Form and have your provider complete it. Then, schedule an appointment at a St. Luke’s Know Your Numbers Screening location for a tobacco screen.

Program Alternatives

St. Luke’s Healthy U is committed to helping you achieve your best health status. Occasionally, it may be appropriate to modify the Healthy U targets. For example, certain health conditions may make it unreasonably difficult to satisfy, or medically inadvisable to attempt to satisfy, the health targets. In such circumstances, St. Luke’s Healthy U will provide you with reasonable alternatives to qualify for the premium reductions, of if medically necessary, waivers for the Healthy U program requirements. St. Luke’s will modify program requirements whenever necessary to comply with federal, state, or local law. Your personal healthcare provider will need to complete and submit a Written Request for Alternative or Waiver.

Privacy Policy

All personal health information (PHI) is protected by the Health Insurance Portability and Accountability Act (HIPAA) and will be safeguarded. Any use of your PHI is covered by St. Luke’s Employee Health Care Plan Notice of Privacy Practices. The number of Healthy U targets you achieve will be reported to our benefits department to assign health plan premiums. Based on results of the KYN Screening and online health assessment, a Healthy U coach may contact you to let you know about available benefits and resources to improve your health.
