MyChart Proxy Access Instructions

For Children Under the Age of 14

In order to set up proxy access for a child's medical record, you must first have your own MyChart account. If you need a MyChart account, please signup online.  

To set up proxy access to your child’s medical record, please submit the request directly from your MyChart account. NOTE: You must use the web browser version of MyChart to set up proxy access, as proxy request is not currently available in the MyChart mobile application.

  1. Access the Menu > Proxy Request Forms
  2. Read and accept the proxy access terms and conditions 
  3. Submit the “Minor” proxy request form. 

Please allow one business day to complete the request. Your proxy access to your child’s medical record will appear in "My Family's Records" the next time you log in to MyChart. 

For Children Between 14 and 17 Years of Age

If your child is between 14 and 17 years of age, your child must set up their own MyChart account. 

  • When a child turns 14, proxy access is automatically restricted to protect the child’s right to privacy regarding information that may be confidential under State and Federal laws. Proxy access may continue if the child completes a Parental Proxy access form, or in other situations where the minor does not have legal capacity. 
  • A child between 14 and 17 years of age can authorize an adult proxy access to their medical record from their own MyChart account. NOTE: You must use the web browser version of MyChart to set up parental proxy access, as this proxy request is not currently available in the MyChart mobile application.

    1. Access the Menu > Proxy Request Forms 
    2. Read and accept the proxy access terms and conditions
    3. Submit the “Adolescent” proxy request form  
Please allow 1 business day to complete the request.

For an Adult

If the patient is able to use MyChart, they can grant proxy access to their MyChart medical record to another adult who must also have a MyChart account. NOTE: You must use the web browser version of MyChart to set up proxy access, as proxy request is not currently available in the MyChart mobile application.

  1. Go to Your Menu  > Share My Record 
  2. Under Who Can See My Record?, click Invite Someone
  3. Follow the instructions to send an email invitation to the person who you'd like to have access to your MyChart account. That person will need to accept the invitation and confirm that they know your date of birth.
Once access is granted, access to your medical record will appear in the proxy section of your proxy's MyChart account.
