Making an Appointment

To make an appointment for your child, we first must receive a referral from your primary care physician. Once we receive the referral, we’ll contact you to verify correct information. You’ll then be mailed paperwork to complete. You may also download the Intake Packet. Once the paperwork is returned to our office, you’ll be contacted to schedule an appointment.

If additional information is needed, we may send behavioral questionnaires for you to complete and return to our office before your scheduled appointment.

General Information

Patients are seen by appointment only. Please arrive 10 minutes early. Please bring your insurance card, photo ID, and current list of medications with you to each visit, and notify us of any changes in your address, phone number, or insurance information. If you’re more than 10 minutes late, we will have to reschedule your appointment.

The doctor needs your full attention during your appointment. If at all possible, we ask that you bring only the child we will be evaluating. However, we understand that this is not always possible. If other family members are present, we would appreciate it if they can wait in the main lobby and play area (adult supervision is required for children in the play area). Please also turn off your cell phones during your visits with us.

New patient visits are approximately 90 minutes, and follow-up visits are 30 minutes. Please plan accordingly.

Please notify us at least 24 hours in advance if you won’t be able to make your scheduled appointment. To avoid no-shows, we require a confirmation phone call from you in response to our reminders. Any appointment not confirmed or canceled will be considered a no-show. This will enable us to serve all patients in a timely manner.
