The single best way to prevent influenza (flu) is to get vaccinated each fall. St. Luke's offers employers a cost-effective way to immunize employees, and safe-guard their health, by providing access to flu shot clinics.

Download this Sign-up Sheet and write the dates/times of your clinic at the top. We will contact you a week prior to the event to confirm the number of participants. This allows us to prepare supplies and staff the clinic appropriately.
Attention Employers: We rely on hospital suppliers to deliver adequate vaccine in a timely manner. Normally the vaccine arrives in mid September although we have experienced delays and shortages in the past. Please note that St. Luke's will make every effort to keep you informed and accommodate your needs.
