The single best way to prevent influenza (flu) is to get vaccinated each fall. St. Luke's offers employers a cost-effective way to immunize employees, and safe-guard their health, by providing access to flu shot clinics.

Flu Shot Voucher Program
The best way to combat the negative effects of the flu season is to provide your employees a cost-effective and convenient way to get vaccinated! Our Flu Shot Voucher program allows you to provide flu vaccinations to your employees at a discounted rate.

Participating in the Flu Shot Voucher Program is as easy as 1-2-3!

1. Get Your Vouchers! You will receive Flu Shot Vouchers once you have registered: Flu Shot Voucher Registration Form.
2. Get the word out! Download the Flu Flyer to promote the flu shot clinic schedule and encourage employees to participate.

3. Give out the Vouchers! Flu shots will be given only to those who present a Flu Shot Voucher. We will not accept any cash, check or credit card payment.

OTHER IMPORTNANT INFORMATION:

• The cost for the flu shot will be discounted to $20 per vaccination through the Flu Shot Voucher Program.
• Employers will pre-order a specified number of Flu Shot Vouchers. Flu Shot Vouchers may be "redeemed" at any flu shot clinic location (see above).
• St. Luke's will invoice the employer for only those vouchers that were redeemed.
• This program is only available to employees of each participating employer. There will be no exceptions made for spouses or dependents.

Attention Employers:
We rely on hospital suppliers to deliver adequate vaccine in a timely manner. Normally the vaccine arrives in mid September although we have experienced delays and shortages in the past. Please note that St. Luke's will make every effort to keep you informed and accommodate your needs.
