Schedule a seasonal flu shot clinic at your worksite!
The single best way to prevent influenza (flu) is to get vaccinated each fall. St. Luke’s Health Solutions offers employers a cost-effective way to immunize employees, and safe-guard their health, by providing onsite flu shot clinics.

H1N1 Flu
St. Luke’s Health Solutions will make every effort to keep you informed of the latest H1N1 developments and recommendations from the Centers for Disease Control and Prevention. They have developed guidelines, checklists and a toolkit to assist employers in planning for a pandemic outbreak. Check the website often at www.flu.gov/plan/workplaceplanning/index.html

The information below is only for the seasonal flu. At this time H1N1 is not available through our onsite vaccination clinics.

Hosting an onsite flu shot clinic is as easy as 1-2-3!

1. Review the General Information for important details on participation, scheduling and pricing. 
2. Complete the Registration Form to reserve a date and time.  
3. Download the following materials for a successful event:

   Get Informed!
   All participants receiving a flu shot will need to read the Vaccine Information Statement (VIS).

   Get Permission!  
   All participants receiving a flu shot will need to complete and sign the Vaccine Consent Form.

   Advertise Your Event!
   Add the flu shot clinic date, time and location to one of the flyer templates provided and post it in a visible area or email it to employees (.doc files)

   Sign Them Up!  
   Add the dates/times of your clinic(s) to the Sign-up Sheet and register employees. We will contact you a week prior to the event to confirm the number of participants. This allows us to staff the clinic appropriately.
