Pre-Placement Evaluation
St. Luke's Occupational Health Service's pre-placement evaluation helps determine if a job applicant is medically qualified to perform the job tasks of a specific job, with or without accommodation.
Pre-Placement Evaluation Includes:
- An examination focused on job demands and work environment
- A comprehensive occupational health history obtained and reviewed by a health care provider
- Determination of medical contraindications in the position offered
- Recommendations for work restriction and accommodation when indicated
- Option: Inclusion of a pre-placement functional screening performed by physical and occupational therapists to determine if the applicant is physically able to perform the essential job functions. This screening uses objective assessment through the use of work task simulation.
- Pre-placement medical clearance status form faxed to the appropriate company contact
- Pre-placement exams completed following your protocols

- Department of Transportation (DOT) physicals
- Hearing screening
- Respirator use evaluation
- Immunizations (hepatitis B, tetanus, influenza)
- Laboratory studies
- Drug and alcohol screening
