Home Care

Specialized, Compassionate Care…in the Home

St. Luke’s Home Care team of skilled professionals provides comprehensive health care in the warmth and comfort of the patient’s home. From newborns to seniors, we offer the latest home care techniques and technology to people at all ages and stages of life.

Our services are provided under a physician’s order for skilled nursing and rehabilitation services. People with acute or chronic disease or other health conditions have different needs, as do their family members. In partnership with the patient, family and physician, our skilled home care professionals create a plan of care tailored to meet the individual needs of each patient.

St. Luke’s Hospice

In cases of life-limiting illness, Hospice services may be appropriate. St. Luke’s Hospice team includes nurses, social workers, chaplains and volunteers dedicated to helping people die with dignity at home, free of pain and surrounded by loved ones.

Scheduling Home Care Visits

St. Luke's Home Care visits are scheduled upon referral from doctors. Once a physician referral is made, a Home Care representative meets with the patient to discuss needs and develop a plan of care.

Insurance Considerations

Home Care is covered by Medicare, Medicaid and most private insurance companies; these agencies are billed directly. For those without insurance coverage, other options may be available.
