Know Your Numbers Screening

New employees will complete the Know Your Numbers (KYN) Screening during the on-boarding process or at Employee Health. Screening consists of biometric measures such as blood pressure, height, weight, body mass index (BMI), waist circumference and fasting blood glucose. Employees/spouses with diabetes will need to provide an A1c lab result within 6 months of hire or benefits eligibility. Additionally, all employees will verify their tobacco status for the past 3 months.

KYN Screening Locations for Your Spouse

You can earn an additional $130 credit when your spouse, who is enrolled in the health plan, also enrolls in St. Luke’s Healthy U within 30 days of your hire date or benefits eligibility. Spouses simply need to complete the same two requirements as you – a Know Your Numbers (KYN) Screening AND online Health Assessment.

KYN Screening appointments are required! The KYN Screening is FREE, but please identify yourself as a St. Luke’s spouse when calling to inquire about an appointment. See below.

Healthcare Provider’s Office
The KYN Screening may be completed by your spouse’s healthcare provider if the visit was within 6 months of your hire date or benefits eligibility. Click here to download the Screening Verification Form. The provider needs to complete the form and fax it to Employee Health Solutions in Boise. Your spouse will also need a copy of the form to complete the online Health Assessment.
