Total Health in the Emergency Department - ThED

The purpose of ThED was to identify a standard, scalable model to reduce preventable ED utilization through resource connection.

Previous studies have indicated that a small percentage of patients often account for a disproportionately large share of health care costs through high utilization of the emergency department (ED). These patients typically have greater needs beyond their acute health condition. 

There is evidence that teams providing support to high-utilization patients can lead to reduced costs, decreased ED visits, and decreased hospital admissions.
