Tuesdays and Thursdays: Same-day appointments for urgent needs are available every Tuesday and Thursday from 5-6 p.m., alternating between our Diamond Street office and our 3165 E. Greenhurst office. Please call (208) 463-7300 or (208) 463-7330 to find out which location is available.

Family Medicine doctors are trained to care for patients of all ages, from infancy through the end of life. We promote the concept of a Medical Home, emphasizing a continuous relationship with a personal physician coordinating care for both wellness and illness. We believe that knowing you over many years, and knowing your family, helps us understand how your health care impacts your life.
St. Luke’s Family Medicine physicians diagnose and treat most medical issues, from infections and sprains to diabetes and asthma. We also specialize in preventive care to help you create and maintain a healthy lifestyle. And when your condition requires a specialist, we can coordinate your care with other physicians and can help you navigate your way through today’s health care.

|Jaiyeola Ashaye, MD is board certified in family medicine, and has special interests in women's health, adolescent and pediatric care, preventive care, and management of chronic medical problems. Dr. Ashaye earned her undergraduate degree from the University of Ibadan, Oyo State, Nigeria and her medical degree at the College of Medicine at the University of Ibadan, Oyo State, Nigeria. She completed her residency in 2002 through the Albert Einstein College of Medicine, Department of Family and Social Medicine at Montefiore Medical Center in Bronx, New York.
|Austin J. Graham, MD is board certified in family medicine, and has special interests in adult medicine, pediatrics, and office procedures. Dr. Graham earned his undergraduate degree from Brigham Young University in Provo, Utah and his medical degree from the Medical College of Ohio in Toledo. He completed his residency at the Washington Hospital Practice Residency Program in Washington, Pennsylvania.
|Jay Hansen, MD is board certified in family medicine. He speaks Spanish, and has special interests in pediatrics, sports medicine, nutrition, weight loss, and diabetes. Dr. Hansen earned his undergraduate degree from Weber State University in Ogden, Utah in 1999 and his medical degree from Tulane University in New Orleans in 2005. He completed his residency through the Mayo School of Graduate Medical Education in Rochester, Minnesota in 2008.
|Stephen J. Zeimantz, NP has several years of experience in family medicine, including the management of acute and chronic illness. He earned his undergraduate degree in nursing from Arkansas Tech University and his Master of Science in Nursing degree from Graceland University.
We try to stay on time, as a courtesy to you and others. Since paperwork may take a few minutes, please arrive early. As emergencies do occur, there may be occasions when your appointment time will be delayed. We will always try to keep you informed in the event of a delay, and will be happy to reschedule your appointment, if you prefer. We ask that you respect the appointment time we have reserved for you, and call 24 hours ahead to cancel or change appointments so that others may be helped during that time.
Communication with your provider is extremely important. It may be helpful for you to write down questions for your doctor or nurse practitioner prior to your visit. Bring a list of medications and the dosages, or bring the bottles with you.
Make sure you understand any instructions you are given. If clarification is needed, review your instructions with the provider or medical assistant before you leave. If you aren’t doing well after your visit, we want to hear from you. Our goal is to improve your health.
We accept most insurance plans, including Medicare, in-state Medicaid, Tricare, Blue Cross/Blue Shield, and others.
Talk with your provider at each visit to be sure your prescriptions are up to date. For refills, contact your pharmacy. Your pharmacy will then contact our office for verification and approval. For patient safety, narcotics cannot be refilled outside of office hours. Talk with your provider before your prescription runs out, and please allow 72 business hours for refills.
