Fridays: Same-day appointments for urgent needs are available every Friday from 5-6 p.m., alternating between our Greenhurst office and our 824 S. Diamond Street office. Please call (208) 463-7330 or (208) 463-7300 to find out which location is available.

Saturdays: Same-day appointments are available every Saturday from 9 a.m.-1 p.m. at our Greenhurst location. Please call (208) 463-7330.

Family Medicine doctors are trained to care for patients of all ages, from infancy through the end of life. We promote the concept of a Medical Home, emphasizing a continuous relationship with a personal physician coordinating care for both wellness and illness.  We believe that knowing you over many years, and knowing your family, helps us understand how your health care impacts your life.

Our Medical Providers


Mark Cheney, DO Mark Cheney, DO is a family medicine physician providing primary health care to patients of all ages, from infants through seniors. He earned his undergraduate degree from Boise State University and his medical degree from Des Moines University. He completed his residency at Firelands Regional Medical Center in Sandusky, Ohio.
Brian Cothern, MD Brian Cothern, MD is board certified in family medicine, and has special interests in musculoskeletal disorders, adult medicine, and pediatrics. Dr. Cothern earned his bachelor's degree from California State University, Chico in 1983 and his medical degree from Uniformed Services University of the Health Services in 1988. He completed his residency at Womack Army Medical Center in Fayetteville, North Carolina in 1991 and was a member of the faculty at the residency program at Womack Army Medical Center from 1993-1997.
Adebayo Crownson, MD Adebayo Crownson, MD is board certified in family medicine, and has special interests in adult medicine, chronic disease management, musculoskeletal disorders, pediatrics, and newborn care. He earned his Bachelor of Science in Nursing degree from Rutgers University in New Brunswick, New Jersey and his medical degree from the University of Medicine and Dentistry in Newark, New Jersey. He completed his residency at Oregon Health & Science University in Portland. Dr. Crownson served as a flight surgeon for the 391st Fighter Squadron in Mountain Home from 1999-2002.
John A. Johnson, MD John A. Johnson, MD is board certified in family medicine and fellowship trained in primary care sports medicine, and is a Fellow of the American Academy of Family Physicians. He has special interests in sports medicine and musculoskeletal disorders. Dr. Johnson earned his bachelor's degree from the U.S. Military Academy in 1976 and his medical degree from Uniformed Services University of the Health Sciences in 1985. He completed his residency at Silas B. Hays Army Community Hospital in Fort Ord, California in 1988 and his sports medicine fellowship at Arlington Sports Medicine Institute in Arlington, Virginia in 1994.
Tricia L. Keefe, DO Tricia L. Keefe, DO is board certified in family medicine and has special interests in women's health, chronic disease management, adolescents, pediatrics, and geriatrics. Dr. Keefe earned her bachelor's degree from Rocky Mountain College in Billings, Montana and her medical degree from Kirksville College of Osteopathic Medicine in Kirksville, Missouri. She completed her residency at the University of Nebraska Medical Center in Omaha in 2000.
Stephen Zeimantz, NP Stephen J. Zeimantz, NP has several years of experience in family medicine, including the management of acute and chronic illness. He earned his undergraduate degree in nursing from Arkansas Tech University and his Master of Science in Nursing degree from Graceland University.

Your Appointment
We try to stay on time, as a courtesy to you and others. Since paperwork may take a few minutes, please arrive early. As emergencies do occur, there may be occasions when your appointment time will be delayed. We will always try to keep you informed in the event of a delay, and will be happy to reschedule your appointment, if you prefer. We ask that you respect the appointment time we have reserved for you, and call 24 hours ahead to cancel or change appointments so that others may be helped during that time.

Communication with your provider is extremely important. It may be helpful for you to write down questions for your doctor or nurse practitioner prior to your visit. Bring a list of medications and the dosages, or bring the bottles with you.

Make sure you understand any instructions you are given. If clarification is needed, review your instructions with the provider or medical assistant before you leave. If you aren’t doing well after your visit, we want to hear from you. Our goal is to improve your health.


Insurance Plans
We accept most insurance plans, including Medicare, in-state Medicaid, Tricare, Blue Cross/Blue Shield, and others.


Medication Refills
Talk with your provider at each visit to be sure your prescriptions are up to date. When you need to refill your prescriptions, you can request them directly through your myChart account.

You can also request a refill by contacting your pharmacy. Your pharmacy will then contact our office for verification and approval. For patient safety, narcotics cannot be refilled outside of office hours. Talk with your provider before your prescription runs out, and please allow 48 hours for refills.
