A physician is always available on call. If you have a medical emergency, please call 911 or go directly to the Emergency Department. For phone questions, call your Family Medicine's office at (208) 473-3500 and you will be connected to the on-call physician.
What is Family Medicine?
Family Medicine doctors are trained to care for patients of all ages, from infancy through the end of life. We promote the concept of a Medical Home, emphasizing a continuous relationship with a personal physician coordinating care for both wellness and illness. We believe that knowing you over many years, and knowing your family, helps us understand how your health care impacts your life.
St. Luke's Family Medicine physicians diagnose and treat most medical issues, from infections and sprains to diabetes and asthma. We also specialize in preventive care to help you create and maintain a healthy lifestyle. And when your condition requires a specialist, we can coordinate your care with other physicians and can help you navigate your way through today's health care.

|Sarah Gerrish, MD is board certified in family medicine. Her professional interests include newborn, pediatric, and adolescent care, obstetrics, women's health, minor surgical procedures, refugee health, and building long-term relationships with families. Dr. Gerrish earned her Bachelor of Science degree in cell and molecular biology and Bachelor of Arts degree in photography in 2001 from the University of Washington, where she also earned her medical degree in 2009. She completed her residency at the Family Medicine Residency of Idaho.
|Coleen Hulbert, MD is board certified in family medicine, and takes pride in establishing long term relationships with her patients. She practices the entire scope of family medicine from pediatrics to geriatrics, with special interest in women's health and preventive health. Dr. Hulbert earned her Bachelor of Science degree from the University of Central Florida and Master of Public Health degree from the University of South Florida. She earned her medical degree from Saba University School of Medicine and completed her internship and residency training at the University of Florida.
|David Hulbert, MD is board certified in family medicine. He is interested in all aspects of family medicine with special emphasis in complex medical problems, procedures, pediatrics, and preventive medicine.
Dr. Hulbert studied neuroscience at Brigham Young University and earned his medical degree from Saba University School of Medicine. He completed his residency at the University of Florida-Gainesville, where he was chief resident from 2011-12 and served as adjunct clinical professor.
Mont Tolman, DO is board certified in family medicine. He specializes in complete care of the entire family, and in sports medicine, with an emphasis on prevention and healthy lifestyle.
He earned his Bachelor of Science degree from Idaho State University in 1987 and his medical degree from Des Moines University in 1991. He served his residency at the Family Practice Residency of Idaho. Dr. Tolman has been in practice in Boise for more than 20 years, most recently with St. Luke's Clinic – Idaho Family Physicians.
|Robert Franklin, DO has interests in all aspects of family medicine, from babies to grandparents. He joins St. Luke's Clinic – Family Medicine in Eagle in 2014 after many years in practice with St. Luke's in Meridian. Dr. Franklin earned his medical degree from Western University of Health Sciences in Pomona, California, and completed his residency at the Family Practice Residency of Delaware County Memorial Hospital in Drexel Hill, Pennsylvania, a Pennsylvania State University affiliate.
We try to stay on time, as a courtesy to you and others. Since paperwork may take a few minutes, please arrive early. As emergencies do occur, there may be occasions when your appointment time will be delayed. We will always try to keep you informed in the event of a delay, and will be happy to reschedule your appointment, if you prefer. We ask that you respect the appointment time we have reserved for you, and call 24 hours ahead to cancel or change appointments so that others may be helped during that time.
Communication with your provider is extremely important. It may be helpful for you to write down questions for your doctor or nurse practitioner prior to your visit. Bring a list of medications and the dosages, or bring the bottles with you.
Make sure you understand any instructions you are given. If clarification is needed, review your instructions with the provider or medical assistant before you leave. If you aren’t doing well after your visit, we want to hear from you. Our goal is to improve your health.
We accept most insurance plans, including Medicare, in-state Medicaid, Tricare, Blue Cross/Blue Shield, and others.
Talk with your provider at each visit to be sure your prescriptions are up to date. When you need to refill your prescriptions, you can request them directly through your myChart account.
You can also request a refill by contacting your pharmacy. Your pharmacy will then contact our office for verification and approval. For patient safety, narcotics cannot be refilled outside of office hours. Talk with your provider before your prescription runs out, and please allow 48 hours for refills.
