After-hours and Weekends
If you feel that you are having an emergency, please proceed to the nearest hospital or call "911." Upon arrival at the hospital, inform the staff that you are a patient of St. Luke's Idaho Cardiology Associates.

If you have a question or concern during office hours, call our office and one of our associates will assist you. If you have an urgent concern that needs immediate attention after office hours, please call our office at (208) 322-1680 and our answering service will page the on-call cardiologist. The cardiologist will return your call after being contacted by the answering service

What is Cardiology?
Physicians with St. Luke's Idaho Cardiology Associates specialize in treating diseases and disorders that affect the heart and its associated blood vessels. Each physician has received advanced education and experience in this specialty, and all are board certified and fellows in the American College of Cardiology. They work with our nurse practitioners and associates staff to provide you with high quality, comprehensive cardiac care.

Our Medical Providers

	Samual P. Bass, MD, FACC is board certified in cardiovascular disease, internal medicine, and nuclear cardiology, and is a fellow of the American College of Cardiology. He earned his medical degree from the University of New Mexico School of Medicine, where he also completed his residency. Dr. Bass joined St. Luke's Idaho Cardiology Associates in 2006, and sees patients at the Boise, Meridian, and Nampa locations and the Weiser regional clinic.
	Andrew U. Chai, MD, FACC earned his medical degree from Medical College of Wisconsin, where he also completed his residency. Dr. Chai is board certified in cardiovascular disease with a subspecialty in nuclear cardiology, and is a fellow of the American College of Cardiology. He joined St. Luke's Idaho Cardiology Associates in 1999. Dr. Chai sees patients at the Caldwell, Meridian, Nampa, and Eagle locations and the Sun Valley regional clinic.
	Frederick M. Costello, MD, FACC is an interventional cardiologist with additional interest in peripheral vascular disease. He is board certified in interventional cardiology, cardiovascular disease, and nuclear cardiology, and is a fellow of the American College of Cardiology. He earned his medical degree from the University of Washington School of Medicine and completed his residency at the University of North Carolina. Dr. Costello joined St. Luke's Idaho Cardiology Associates in 2008, and sees patients at the Meridian, Eagle, and Nampa locations.
	Robert L. Duerr, MD, FACC is board certified in interventional cardiology and cardiovascular disease, and is a fellow of the American College of Cardiolog. He earned his medical degree from Harvard Medical School and completed his residency at Bellevue Hospital, New York University. Dr. Duerr joined St. Luke's Idaho Cardiology Associates in 1999, and sees patients at the Boise and Meridian locations and the Sun Valley regional clinic.
	Charles E. Eiriksson, MD, FACC is board certified in interventional cardiology and cardiovascular disease, and is a fellow of the American College of Cardiology. He earned his medical degree from the University of Washington School of Medicine and completed his residency at the University of Utah Affiliated Hospitals. Dr. Eiriksson is a founding member of what was then Idaho Cardiology Associates, which was formed in 1994. He sees sees patients at the Boise and Meridian locations and the Sun Valley regional clinic.
	Stefanie J. Fry, MD, FACC is board certified in cardiovascular disease and nuclear cardiology, and is a fellow of the American College of Cardiology. She earned her medical degree from the University of Illinois College of Medicine and completed her residency at the University of New Mexico Health Sciences Center. Dr. Fry joined St. Luke's Idaho Cardiology Associates in 1999, and sees patients at the Boise and Meridian locations and the Baker City, McCall, and LaGrande regional clinics.
	Nathan Green, MD, FACC is board certified in interventional cardiology and cardiovascular disease, and is a Fellow of the American College of Cardiology. He earned his medical degree with honors from the University of Washington School of Medicine. He completed his residency at Duke University Medical Center and his fellowships in cardiology and interventional cardiology at the University of Colorado Health Sciences Center. Before joining St. Luke's in 2013, he served as medical director of Cardiovascular Services for Group Health in Washington State. Dr. Green sees patients at the Jefferson Street and Meridian clinics.
	Kristin M. Linzmeyer, MD, FACC is board certified in internal medicine, cardiology, and nuclear cardiology. She earned her bachelor's degree from Carroll College and her medical degree from Oregon Health & Science University, where she also performed her internal medicine residency. She completed her internal medicine internship at the University of Utah and her cardiology fellowship at the University of New Mexico. Dr. Linzmeyer sees patients at the Meridian, Eagle, Nampa, and Caldwell locations and at regional clinics in Mountain Home, Baker City, and LaGrande.
	Melinda L. Marks, MD, FACC is a cardiologist and cardiac electrophysiologist. She is board certified in clinical cardiac electrophysiology and cardiovascular disease, and is a fellow of the American College of Cardiology. She earned her medical degree from the University of Washington School of Medicine and completed her residency at the University of Washington Medical Center. Dr. Marks joined St. Luke's Idaho Cardiology Associates in 1996, and sees patients at the Caldwell, Jefferson, and Meridian locations and the McCall regional clinic.
	Brian P. Nolan, DO, FACC is board certified in internal medicine, cardiovascular disease, and nuclear cardiology. He specializes in nuclear cardiology and heart failure. He earned his medical degree from Arizona College of Osteopathic Medicine, Midwestern University. He completed his internal medicine residency and cardiovascular fellowship at Oregon Health & Science University. Dr. Nolan joined St. Luke's Idaho Cardiology Associates in 2010, and sees patients at the Nampa , Meridian, and Caldwell locations and the Baker City and LaGrande regional clinics.
	Marshall F. Priest, MD, FACC is board certified in interventional cardiology and cardiovascular disease, and is a fellow of the American College of Cardiology. He earned his medical degree from the University of Tennessee, where he also completed his residency. Dr. Priest is a founding member of what was then Idaho Cardiology Associates, which was formed in 1994. He sees patients at the Meridian and Eagle locations and the Baker City and LaGrande regional clinics.
	Karl P. Undesser, MD, PhD, FACC is a cardiologist and cardiac electrophysiologist. He is board certified in clinical cardiac electrophysiology and cardiovascular disease, and is a fellow of the American College of Cardiology. He earned his medical degree from the University of Texas Health Science Center and completed his residency at Duke University Medical Center. Dr. Undesser joined St. Luke's Idaho Cardiology Associates in 1999, and sees patients at the Boise and Meridian locations.
	Katie Assmus, PA-C is a certified physician assistant. She earned her bachelor's degree from Whitworth University and her master's degree in physician assistant studies from Idaho State University. She sees patients at the Meridian location.
	Meghann Bostrom, NP-C earned her Bachelor of Science in Nursing degree from Washington State University and her Master of Science in Nursing degree, nurse practitioner, from Gonzaga University. She sees patients at the Meridian location.
	Molly Lynn, FNP is a certified family nurse practitioner by the American Nurse Credentialing Center. She earned her Bachelor of Science in Nursing degree from Boise State University and her Master of Science in Nursing degree, family nurse practitioner, from Cincinnati University.
	Anna McCreath, NP-C earned her Bachelor of Science in Nursing degree from Idaho State University and her Master of Science in Nursing degree from Gonzaga University. Anna joined St. Luke's Idaho Cardiology Associates in 2003, and sees patients at the Caldwell and Meridian locations.
	Brad Mulkey, PA-C is a certified physician assistant. He earned his bachelor's degree in respiratory therapy from Boise State University and his master's degree in physician assistant studies from the University of Utah School of Medicine. He sees patients at the Meridian location.
	Dana Peachey, NP-C earned her Bachelor of Science in Nursing degree from Boise State University and her family nurse practitioner degree from Gonzaga University. Dana joined St. Luke's Idaho Cardiology Associates in 2005. She sees patients at the Boise and Meridian locations and the Union regional clinic.

Your Appointment
We try to stay on time, as a courtesy to you and others. Since paperwork may take a few minutes, please arrive early. As emergencies do occur, there may be occasions when your appointment time will be delayed. We will always try to keep you informed in the event of a delay, and will be happy to reschedule your appointment, if you prefer. We ask that you respect the appointment time we have reserved for you, and call 24 hours ahead to cancel or change appointments so that others may be helped during that time.

Communication with your provider is extremely important. It may be helpful for you to write down questions for your doctor or nurse practitioner prior to your visit. Bring a list of medications and the dosages, or bring the bottles with you.

Make sure you understand any instructions you are given. If clarification is needed, review your instructions with the provider or medical assistant before you leave. If you aren’t doing well after your visit, we want to hear from you. Our goal is to improve your health.

Please bring to your appointment:

• A complete and up-to-date list of all current medications with dosage information.
• All pertinent insurance information and your insurance cards; and let us know if any information changes between visits.

Insurance Plans
We accept most insurance plans, including Medicare, in-state Medicaid, Tricare, Blue Cross/Blue Shield, and others.

The list is subject to change; please give our office a call to inquire about coverage.

As a courtesy, our practice routinely files primary and supplemental insurance claims for our patients. Insurance may pay for all, some, or none of your bill. Please remember that you are responsible for payment of balances not covered by your insurance. You will receive a monthly statement from St. Luke's Idaho Cardiology Associates as long as your account is active. For questions concerning insurance or account balances not covered by insurance, please call

Medication Refills
Talk with your provider at each visit to be sure your prescriptions are up to date. For refills, contact your pharmacy. Your pharmacy will then contact our office for verification and approval. For patient safety, narcotics cannot be refilled outside of office hours. Talk with your provider before your prescription runs out, and please allow 48 hours for refills.
