Required Scheduling Information

To schedule an appointment by phone, please have the following information available at the time of scheduling.

Please Note: Preauthorization is required when scheduling MRI, CT and Nuclear Medicine “PET scans”. If scheduling multiple body parts, each body part requires a separate authorization number. (Preauthorization numbers are to be obtained by the ordering provider prior to calling St Luke’s Central Scheduling and/or Imaging Center).

If patient is a "Healthy Connections" referral, please provide information at scheduling and send referral letter with patient.

If patient needs an interpreter please inform the scheduling agent at the time of booking. They’ll be happy to arrange for an interpreter “at no charge to the patient.”
