Home Care Services

Our Pediatric Care Team provides a high level of skilled care to meet the special needs of the pediatric patient in the home setting. Along with oncology, orthopedic, cardiac and IV therapy patients, our multidisciplinary team provides specialized home care services for the premature infant, children of high-risk families, children with congenital anomalies and swallowing and feeding problems. An individualized family-centered plan of care is developed for each patient admitted to Home Care. Our providers’ cumulative years of dedicated pediatric care in nursing, social work and physical, occupational and speech therapy provide a continuum of care as the child transitions to and from the home environment.
