Information Needed to Book a Case
- Provider full name
- Procedure (Please be specific and as accurate as possible)
- Date and time you prefer
- Length of time needed to complete procedure (Approximate duration of the procedure – our computer system averages the surgeon’s previous case times and we will use that average if available.)
- Anesthesia type
- Pre-case diagnosis
- Type of patient in-house, AM admit, observation, or outpatient
Patient Information
- Social Security number
- Name and correct spelling
- Date of birth
- Accurate contact phone number
Fax Surgeon Orders to the Pre-Surgery Clinic( if utilized)
at 381-3060 or to the Surgery Center at 381-3209.
If your case requires any of the following, please let us
know at the time of booking as they require additional planning
to meet your needs:
- Special instrumentation and equipment
- The Laser
- C-Arm
If your patient requires an Interpreter, please let us know what language or dialect is needed when you schedule the case.
