Information Needed to Book a Case
- Provider Full Name
- Procedure (Please be specific and as accurate as possible)
- Date & Time You Prefer
- Length of Time Needed to Complete Procedure (Approximate duration of the procedure. Our computer system averages the surgeon’s previous case times. We will use that average if available.)
- Anesthesia Type
- Pre-Case Diagnosis
- Type of Patient In-house, AM Admit, Observation or Outpatient
Patient Information
- Social Security Number
- Name & Correct Spelling
- Date of Birth
- Accurate Contact Phone Number
Fax Surgeon Orders to the Pre-Surgery Clinic( if utilized)
at 381-3060 or to The Surgery Center at 381-3209.
If your case requires any of the following, please let us
know at the time of booking as they require additional planning
to meet your needs:
- Special Instrumentation and Equipment
- The Laser
- C-Arm
If your patient requires an Interpreter, please let us know what dialect is needed when you schedule the case.
